Licensed Practical Nurse (LPN) – Client Care & Education

Home Instead is looking for a compassionate, knowledgeable, and personable Licensed Practical Nurse (LPN) to join our Client Care Team in Vancouver.

This is a unique opportunity for an LPN who enjoys supporting clients and families, educating and mentoring CAREGivers, collaborating with a care team, and sharing their knowledge within the community.

As a member of our Client Care Team, you will serve as an important clinical resource and educator, helping our team provide safe, consistent, and high-quality care to older adults in their homes.

This role is ideal for an LPN who enjoys building relationships and using their clinical knowledge in a community-based, supportive, and collaborative environment rather than a traditional clinical setting.

What You'll Do

Client Care & Clinical Support

• Work closely with the Client Care Team to support the coordination and delivery of quality care to our clients.

• Participate in client assessments, care planning, and quality assurance visits as needed.

• Monitor and document changes in clients' physical, cognitive, emotional, and functional needs.

• Provide clinical guidance to the Client Care Team when concerns or changes in a client's condition arise.

• Support the development and ongoing review of individualized care plans.

• Help identify potential risks and opportunities to improve client safety and quality of care.

• Communicate with clients, families, CAREGivers, and other members of the care team regarding client needs and concerns.

• Provide guidance related to medication support and other care-related questions within the scope of practice.

CAREGiver Education & Support

• Provide ongoing education, coaching, and clinical support to CAREGivers on site or as needed. 

• Help CAREGivers understand and respond appropriately to clients' changing needs.

• Provide specialized education and guidance for CAREGivers supporting clients with higher or more complex care needs.

• Reinforce safe caregiving practices, professional boundaries, documentation, infection prevention, and client safety.

• Provide guidance and support to CAREGivers in the field when additional assistance is required.

• Identify training needs and contribute to ongoing CAREGiver education and development.

Community Education & Engagement

• Represent Home Instead at community events, educational sessions, and partner activities.

• Deliver educational presentations on topics related to aging, caregiving, senior wellness, dementia, fall prevention, health and safety, and other relevant topics.

• Educate seniors, families, caregivers, and community partners about aging and available care resources.

• Build positive relationships with community organizations and health-care professionals.

• Support Home Instead's efforts to increase awareness of resources and services available to older adults and their families.

Team Collaboration & Quality

• Work collaboratively with the Client Care Team, CAREGiver team, and leadership.

• Provide clinical perspective and recommendations to support client care decisions.

• Participate in quality improvement initiatives and help identify opportunities to strengthen our services.

• Contribute to the development and updating of educational resources, care materials, and internal processes.

• Maintain accurate and timely documentation of client, CAREGiver, and educational activities.

• Promote a culture of safety, compassion, accountability, and person-centred care.

What We're Looking For

We are looking for an LPN who combines clinical knowledge with strong communication, teaching, and relationship-building skills.

Qualifications

• Current registration and good standing with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.

• Experience working with older adults, community care, home care, or a related setting is an asset.

• Valid driver's licence with a clean driving record and abstract.

• Strong verbal, written, and interpersonal communication skills.

• Comfortable providing education to individuals and groups.

• Strong organizational and time-management skills.

• Successful completion of reference and background checks.
